If you are getting Social Security, Railroad Retirement, or disability benefits, you'll automatically get enrolled in Medicare Parts A and B. About 3 months before you turn 65 or 24 months into disability, you'll receive an Initial Enrollment Package. This includes info about Medicare, a questionnaire, and your Medicare card. If you want both Part A and Part B, sign your Medicare card and keep it with you. To decline Part B, mark the refusal box on the card, sign, and return it to Social Security. You'll get a new card showing only Part A.
Medicare does NOT cover routine dental care or most procedures like cleanings, fillings, extractions, or dentures. Occasionally, Medicare Part B may cover certain dental services, and Part A might cover some inpatient dental care.
Medicare does NOT cover prescription drugs. However, Medicare Part A will cover drugs received as part of treatment during an inpatient hospital stay.
How can I get a replacement Medicare card?
To get a replacement card, call 1-800-772-1213 or visit the Social Security Administration website. Have your Medicare number on hand when you call. You should expect to receive your new card in approximately four weeks.
If your income is limited, your state might assist with Medicare costs like premiums and deductibles. Contact your State Medical Assistance program and they will be able to determine your eligibility. With Medicare Part A, limited income, and financial resources below $4,000 for an individual or $6,000 for a couple, you may qualify for assistance as a Qualified Medicare Beneficiary (QMB) or Specified Low-Income Medicare Beneficiary (SLMB). QMB covers Part B premiums, deductibles, and coinsurance, while SLMB helps with Part B premiums.
Beneficiaries with higher income could be subject to pay a larger monthly Part B and Part D premium based on there income. The additional amount is referred to as an income-related monthly adjustment amount.

Please contact Medicare.gov, 1-800-MEDICARE, or your local State Health Insurance Program (SHIP) to get information on all of your options. We represent Medicare Advantage [HMO, PPO, and PFFS] organizations and stand-alone PDP prescription drug plans that have a Medicare contract. Enrollment depends on the plan’s contract renewal. Extra benefits require enrollment in an MA plan and depend on whether you are eligible to enroll in an MA plan in your area. Benefits are available only in select areas.
Medicare Supplement insurance is available to those age 65 and older enrolled in Medicare Parts A and B and, in some states, to those under age 65 eligible for Medicare due to disability or End-Stage Renal disease.
